How does it work?
First you need to create a form with any information you wish.
You can create a form here for free.
Next, purchase the code for only $20. The code (ASP) takes the user imput, plus the textfield names, text box names, list menu names, check box names, etc., and submits the infomation to your email address in the same format as appears on the form. It also pre-populates the "reply to" fields with the submitters email address and a subject of your choice. Just read and follow the easy instructions in the zip file, upload, and that's it!
